If you have to write an incident report be sure to have all of the pertinent information to the incident. The report should include all parties involved in the incident, the date and time the incident occurred, a brief description of the incident and who is taking and documenting the report. You should keep a copy as well as turn a copy in to the location where the incident occurred.
