A sworn statement or affidavit should include all of your personal information, as well as your relationship to the case or legal proceedings, i.e. "I am the person who witnessed the accident".

Writing a sworn statement is not difficult to do. Make your statement of fact as clearly and concisely as possible. At either the end or the beginning you should add information stating that you are swearing to the truth of the statement. Sign the statement and have it notarized.
